Mobility Limitation Among Persons Aged 40 Years With and Without Diagnosed Diabetes and Lower Extremity Disease -United States, 1999-2002

40歳以上の下肢疾患合併/非合併糖尿病患者における運動制限－アメリカ、1999～2002年

糖尿病患者は特に高齢者において下肢疾患(LED)による運動制限リスクが高い。CDCは40歳以上の成人に関し、1999－2002年のNational Health and Nutrition Examination Survey(NHANES)データを分析し、LED合併/非合併糖尿病患者における運動制限に関する調査を行った。調査対象は健康診断を受けた40歳以上の成人4,689名(LED/糖尿病:181、非LED/糖尿病:368、LED/非糖尿病:859、非LED/非糖尿病:3,281名)であり、運動制限はLED/糖尿病:38.8%、非LED/糖尿病:22.6%、LED/非糖尿病:24.7%、非LED/非糖尿病:13.8%の症例があると回答し、それぞれの因子別では糖尿病:27.2%、非糖尿病:15.9%、LED:26.4%、非LED:14.5%であった。いずれも40－59歳に比し60歳以上の症例にて高値であり、男女別では女性での運動制限を高率に認めた。運動制限は1/4マイル歩行および階段10ステップにおける障害が多く(LED/糖尿病にて33%)、同階の移動は6%であった。今回の調査では、糖尿患者における運動制限は非糖尿病患者に比し有意に高率に認められたが、運動制限に関与する失明、肥満、循環器系疾患、関節炎などの他因子も含め、さらに詳細な分析が必要と考えられた。
