Lower Extremity Disease Among Persons Aged 40 Years With and Without Diabetes - United States, 1999-2002

40歳以上の糖尿病合併および非合併成人における下肢疾患－アメリカ,1999～2002年

末梢動脈疾患（PAD）や末梢無感覚性ニューロパシー（PN）などを含む下肢疾患（LED）は特に高齢者と糖尿病患者で罹患率の高い慢性病態である。LEDは障害性足合併症（潰瘍、感染症、壊疽、切断など）をもたらし、PADは心血管系疾患の発症リスクと死亡リスクを高める。以前公表された40歳以上の糖尿病合併および非合併成人におけるLED有病率を更新するため、CDCは1999－2002年のNational Health and Nutrition Examination Survey（NHANES）のデータを分析した。40歳以上のアメリカ成人におけるPADの有病率は約5%、PNの有病率は約13%であり、約4%は足潰瘍や足病変、足指/足の切断を経験していた。全体として、約18.6%が一つ以上のLED症状を有しており、その2/3が無症状であった。症候性のPNあるいはLEDの有病率は糖尿病合併例で非合併例に比べ高く、PN患者では糖尿病合併例の42%、非合併例の21%、LED患者では糖尿病合併例の53%、非合併例の31%が症候性であった。LEDの有病率は75歳以上（40.8%）と60－74歳（26.2%）が40－59歳（12.3%）より、男性（23.1%）が女性（16.6%）より、非ラテンアメリカ系黒人（27.0%）が非ラテンアメリカ系白人（19.1%）とメキシコ系アメリカ人（21.1%）より高かった。LEDの年齢調整有病率は糖尿病合併例で非合併例に比べ1.5－1.8倍高かった。LEDの様々な合併症は、LEDの早期発見により予防できる。医師と一般人における糖尿病などLEDの危険因子についての知識を増加させることは、LEDの早期発見や介入、障害性疾患を予防するための治療に有用と考えられた。
