Prevalence of Receiving Multiple Preventive-Care Services Among Adults with Diabetes - United States, 2002-2004

複数の予防措置サービスを受けている糖尿病患者の割合－アメリカ、2002～2004年

アメリカでは7%が糖尿病を患っていると推定されているが、診断されている症例は70%に過ぎない。糖尿病合併症の予防措置として、足や眼の定期的検査、glycosylated hemoglobin(A1C)測定(年2－3回)を受けるよう推奨されており、2010年の健康目標にも挙げられている。今回、CDCは糖尿病患者におけるこれらの予防措置を受けている患者の割合を調査するため、2002－2004年BRFSSデータを分析した(回答率は2002年:58.3%、2003年:53.2%、2004年:52.7%)。その結果、回答者807,771名のうち、「糖尿病と診断されたことがある」と回答した糖尿病患者は65,441名であり、複数の予防措置を受けている患者は39.5%(プエルトリコ20.7%－ハワイ:64.0%)であった。州別ではハワイ(64.0%)、ニューハンプシャー(56.7%)、ミネソタ(55.3%)、マサチューセッツ(54.3%)、サウスダコタ(52.1%)、ウイスコンシン(50.3%)の6州にて半数以上の患者が受けており、年齢別では18－44歳に比べ45歳以上、人種別ではラテン系に比べ非ラテン系白人、現喫煙者に比べ前喫煙者にて高値であり、また、ハイスクール卒業以上、年収5万ドル以上、罹病期間4年以上、インスリン使用者、糖尿病に関する教育を受けている患者、健康保険を有する患者にて高値であった。多変量解析では、サービスを受けている割合と75歳以上、非ラテン系黒人、ハイスクール卒業以上、罹病期間10－19年、インスリン、糖尿病教育および健康保険との有意な正相関、現喫煙者と逆相関が認められた。
