輸入による麻疹の発生－インディアナ州、2005年5～6月

2005年5月29日、Indiana State Department of Health(ISDH)にインディアナ州在住の6歳女児が麻疹の疑いでオハイオ州シンシナティの病院に入院しているとの報告があった。この女児は5月15日、インディアナ州北西部の教会の集会に参加しており、この集会に参加していた17歳女性が麻疹を発症していたことが明らかとなった。この女性は麻疹予防接種を受けておらず、5月4－14日、麻疹の発生地域であるルーマニア、ブカレストの孤児院と病院を伝道師として訪問しており、発熱、咳、結膜炎の前駆症状を呈し14日に帰国、15日に教会の集会に参加した。翌16日に発疹を来たし、麻疹と診断されている。ISDHおよびCDCの調査により計34例［12(9ヶ月－49)歳］が確診され、発疹の発症は5月16日－6月24日、33例が教会の集会に参加あるいは参加していた症例の家族、1例は麻疹症例が入院していたインディアナ州の病院に勤務する瀉血専門医であった。予防接種は2例(6%)が受けていた。血清検査またはRT-PCR法による確診例は14例(41%)、他の20例は確診例と関連のある発疹例であり、入院例は3例(9%)であった。今回の発生は、症例の隔離、症例との接触の可能性の追跡および可能性のあるヒトへのワクチン接種(拒否例は自宅隔離)などの措置によりコントロールされた。1997－2001年におけるアメリカでの麻疹症例540例のうち、輸入症例は362例(67%)を占め、Advisory Committee on Immunization Practice(ACIP)は海外旅行者への麻疹予防接種を勧告している。
