Influenza Vaccination in Pregnancy: Practices Among Obstetrician-Gynecologists - United States, 2003-04 Influenza Season

妊娠中のインフルエンザウイルス感染は、重度の合併症や入院のリスクを高める。1997－2003年、Advisory Committee on Immunization Practices（ACIP）はインフルエンザワクチン接種の適応となるハイリスク群の中にインフルエンザシーズン中に妊娠第2あるいは第3トリメスター期にある健常女性を含めた。また妊娠期に関係なく喘息や糖尿病、心疾患など慢性疾患を有する女性もインフルエンザワクチン接種の適応とした。ACIPは、インフルエンザワクチンは授乳婦や新生児に対しても安全であり、2歳未満の小児と接触する家族もワクチン接種を受ける必要があることを強調している。この勧告にもかかわらず、2003年の妊婦におけるインフルエンザワクチン接種率は13%と低い。2004年5月、American College of Obstetricians and Gynecologists（ACOG）はCDCの支援を受け全国の産婦人科医のうち無作為に選択した413名を対象とし2003年ACIP勧告の理解度について調査を行った。調査対象とした産婦人科医には、2003－04インフルエンザシーズン中の妊婦および産褥婦に対するインフルエンザワクチン接種の勧告とその実施について質問した。全体の52%は11月に妊娠第1トリメスター期にある健常女性に、95%は11月に第2・第3トリメスター期にある健常女性に、63%は11月に第1トリメスター期にある糖尿病女性に、63%は生後3ヶ月の子供がおり授乳中の健常女性に対してワクチン接種を勧めていた。しかし、ワクチン接種を勧めた医師のうち36－38%はワクチン接種を自施設で実施していなかった。健常妊婦およびハイリスク妊婦に対する第1トリメスター期でのインフルエンザワクチン接種率を改善し、産婦人科医に対し最新ACIP勧告について教育するためには、さらなる努力が必要である。
