Use of Social Networks to Identify Persons with Undiagnosed HIV Infection - Seven U.S. Cities, October 2003-September 2004

アメリカではHIV感染生存者約25万人が感染およびHIV感染を媒介する危険性を認識していないことから、CDCのAdvancing HIV Prevention Initiativeの一部として、HIV感染の非認識例を確認し、それらの治療および感染拡大予防介入が国の優先課題とされている。2003年、国内7都市（サンフランシスコ、ワシントン［DC］、オーランド、ラフィエット［ルイジアナ州］、ボストン、ニューヨーク市、フィラデルフィア）の9つの地域的組織(CBOs)プロジェクトがこの問題における社会的ネットワークの利用戦略を評価する目的に開始され、今回、このプロジェクトの1年経過時点での状況がまとめられた。2003年10月－2004年9月、9つのCBOから133名のリクルーター(HIV陽性および陰性ハイリスク例)が参加し、計814名のネットワーク仲間(NAs:リクルーターの社会的、性的、薬物使用関係者)がHIV検査を受けた。リクルーターは男性80、女性43、性転換者10名、HIV陽性ハイリスク:77%、陰性ハイリスク:16%であり、年齢は113例(85%)が35歳以上であり、マイノリティーが125名(94%)［非ラテン系黒人:88(66%)、ラテン系:34(26%)］を占めた。また異性愛ハイリスク:54(41%)、男性同性愛(MSM)ハイリスク:39(29%)、MSM/静注ドラッグ使用者(IDUs):15(11%)、IDUs:12(9%)であった。HIV検査を受けたNAsは、男性544名(67%)、677名(83%)が25歳以上であり、非ラテン系黒人:383名(47%)、ラテン系:310名(38%)であり、検査の結果は、HIV陰性ハイリスク:669名(82%)、HIV陰性低リスク:79(10%)、新規HIV感染例:46(6%)、既診断例:12(2%)、不明:8(1%)であった。新規診断例はMSMDU(26%)、性転換者(20%)およびMSM(16%)にて多く認められた。今回の調査から、CBOによる社会的ネットワーク戦略は効果があると考えられ、今後も継続する価値があると判断される。
