風疹および先天性風疹の根絶－アメリカ,1969～2004年

アメリカおよび西半球における風疹ワクチン接種プログラムの歴史とその成果、今後の風疹根絶戦略について述べる。世界中で風疹が流行していた1962―1965年、アメリカでは約1,250万例が風疹に罹患し、2,000例が脳炎を発症し、胎児11,250例および新生児2,100例が死亡し、乳児20,000例がCRSを有していた。アメリカにおけるこの伝染病の経済的影響は15億ドルと推定された。このため風疹ワクチンの開発が促進され、ワクチンによる風疹予防戦略の必要性が強調された。1969年に風疹ワクチンが認可され、CRSを含む先天性感染症予防を目標としたワクチン接種プログラムが確立された。さらに1977年には小児期のワクチン接種率を90%以上に増加させるための活動が開始された。この活動により、風疹患者の報告数は1977年の20,395例から1981年には2,077例へと激減した。最近のデータによると2001年以降の風疹患者の報告数は過去最低であり、2001年23例、2002年18例、2003年7例、2004年9例であった。このような少ない患者数、就学年齢小児のワクチン接種率が95%以上であること、人口の約91%が免疫性を有していること、風疹のアウトブレイクを検出するための適切なサーベイランスの実施、ウイルス遺伝子型パターンが世界の他の地域で発生したウイルスと一致することより、アメリカにおいて風疹の風土病的伝播はないと考えられている。今後も風疹とCRSに対する予防策として小児における高いワクチン接種率の維持、出産可能年齢の女性(特に出生地がアメリカ以外の女性)における確実なワクチン接種、風疹およびCRSのサーベイランス、アウトブレイクへの迅速な対応を続けていくべきである。
