地域に密着したサ-ビスの提供による小児喘息の減少－ニュ-ヨ-ク市,2001～2004年

アメリカでは1980年以降、喘息の有病率、入院率、死亡率が増加している。ニュ-ヨ-ク市(NYC)は、サ-ベイランス、医療の利用、健康管理サ-ビス提供の改善を通してCentral Harlemの60ブロック圏内に住む12歳以下の小児における喘息関連罹患率を低下させるため、Harlem Children’s Zone Projectとして2001年にHarlem Children’s Zone Asthma Initiative(HCZAI)を設立した。HCZAIでは、スクリ-ニングにて喘息あるいは喘息様徴候、喘息に一致する身体所見がみられた小児の家族に対しプログラムへの参加を勧めている。2004年9月現在、小児3,132人がスクリ-ニングを受け、982例(31.4%)で喘息あるいは喘息様徴候が認められ、314例(10.0%)がHCZAIに登録された。HCZAIの喘息罹病率低下効果に関する予備的デ-タを得るため、18ヶ月の経過観察期間を3ヶ月ずつ6期間に分け、各期間で喘息症状および管理戦略利用状況(全13項目)について評価した。親/保護者から報告された登録小児の調査前14日間の学校欠席率は18ヶ月間で34.4%から16.0%に低下し、喘息による欠席率は23.3%から8%に低下した。さらに喘息治療のための救急部や診療所の緊急受診率は35.0%から8.0%に低下した。これは、プログラム登録者の喘息管理改善と健康管理サ-ビスの適切な利用が原因と考えた。このようなHCZAIの効果は、喘息罹病率低下における地域密着型公衆衛生プログラムの有用性を強調する。
