Acute Hepatitis B Among Children and Adolescents - United States, 1990-2002

小児および思春期年齢における急性B型肝炎－アメリカ、1990～2002年

1991年、B型肝炎ウイルス(HBV)感染の阻止対策が実施され、その後急性B型肝炎の発症は減少している。特に予防接種が推奨された1991年以降に出生した小児での減少は大きく、接種奨励は1995年には11―12歳、1999年には18歳以下の小児に拡大している。1990年以降に出生した小児を対象としたCDCによる急性B型肝炎に関する調査では、1990―2002年の19歳以下の急性B型肝炎症例は13,829例、発症率は10万人あたり1990年3.03から2002年0.34へと89%減少していた。15－19歳の思春期年齢における発症率は1990年8.69、2002年1.13と87%減少したが14歳以下に比べ高値を示した(0―4歳:94%、5―9歳:92%、10―14歳:93%)。人種/民族別では1990年、アジア/太平洋諸島(A/PI):6.74、黒人:4.29にて高く、白人が最も低値であった(1.39)。1990―2002年までの減少率はA/PI:92%、白人:88%、黒人:88%、アメリカインディアン/アラスカ原住民(AI/AN):84%であり、2002年における発症率はA/PI:0.55、黒人:0.51、AI/AN:0.43、白人:0.16の順であった。また、2001―2002年、1990年以降に生まれた小児の発症例(19例)の個別調査では、男児12、女児7例、2歳以下8例(42%)、アメリカ出生児11(58%)であり、人種/民族別ではA/PI:7(37%)、白人:5(26%)、黒人:4(21%)、不明:3(16%)であった。国外での出生例8例のうち、6例は国際養子縁組であった。3例(16%)は1回以上の接種が確認されたが、12例に関しては不明である。急性B型肝炎の発症率は低下しているが、リスク因子同定やサーベイランスの質の向上を目指していく必要がある。
