Postexposure Prophylaxis, Isolation, and Quarantine To Control an Import-Associated Measles Outbreak - Iowa, 2004

輸入による麻疹の流行コントロール目的の曝露後予防、隔離および検疫－アイオワ州,2004年

2004年3月12日、麻疹に感染した大学生がインドからアイオワ州に帰国し、その後2例に感染している。これら3例は隔離され、Iowa Department of Public Health(IDPH)および地元の保健所により訪問または電話によりモニターされている。さらに第2の症例をケアした医療従事者2例での感染が疑われ、同様に2週間隔離された。3例中2例は予防接種率の低い(k-12学校の入学時ワクチン接種率59%)島部の住民(島人口:2,000―3,000人)であり、症例との接触から感染が疑われる地元住民に対して曝露後予防(PEP)が行われ、56名が受けている。PEPを拒否した7名は自宅からの外出を禁じられ、4名は血清学的に免疫が確認され解除されたが、3名は2週間隔離となった。その後新たな症例は報告されていない。伝染性疾患への感染が疑われる症例の隔離に関しては、2003年にSARSが流行した際にCDCが感染拡大の予防を目的に10項目の規定を定めているが、実際に隔離する方法や期間については州によりばらついており、今後見直しが必要である。
