High-Risk Sexual Behavior by HIV-Positive Men Who Have Sex with Men - 16 Sites, United States, 2000-2002

HIV陽性の男性同性愛者によるハイリスクな性行動－16州,アメリカ,2000～2002年

アメリカにおいて、HIV陽性生存者の多くは男性同性愛者(MSM)である。HIV陽性MSMによるハイリスクな性行動は、性パートナーをHIVに曝露させる。2000年5月―2002年12月、アメリカ16州において18歳以上のHIV陽性MSM2,491名を対象としたインタビューによる性行動サーベイランスが実施された。過去12ヶ月の間でHIV陽性と診断された1,923名(77%)中1,177名(61%)は過去12ヶ月間に男性と性交渉を行ったと回答し、586名(31%)は禁欲していたと回答し、160名(8%)は男性以外と性交渉を行ったと回答した。この期間中の男性性パートナー数は平均2名(1―500名)であった。性リスク行動のデータが得られたMSM1,153名のうち422名(37%)は過去12ヶ月間に一定の男性性パートナーと、731名(63%)は不特定な男性性パートナーと関係を持っていた。パートナーのHIV感染状況の認識不足は、性パートナー不特定群(36%)の方が性パートナー一定群(8%)に比べ有意に高かった。性的活動性を有するMSM全体の40%は最近の性交時に肛門挿入性交(UIAI)を行っており、このうち25%はコンドームを使用していなかった。UIAIのパートナーとしてはHIV陰性者および血清状態不明者がHIV陽性者に比べ有意に少なく、また予防しないUIAIのパートナーはHIV陰性者がHIV陽性者より少ない傾向にあった。HIV伝播を遮断するためには、このようなHIV伝播リスクが高い性行動を続けているグループを対象としたより強力な包括的HIV予防活動を行う必要がある。
