Investigation of Acute Idiopathic Pulmonary Hemorrhage Among Infants - Massachusetts, December 2002-June 2003

乳児における急性特発性肺出血の調査―マサチューセッツ州,2002年12月～2003年6月

1993―1996年、オハイオ州クリーブランドにて急性特発性肺出血(AIPH)を発症した乳児の調査では、カビ(Stachybotrys chartarum)、タバコの煙への曝露および母乳不足との関連性が示唆されたが、CDCによるその後の調査ではカビとの関連性は否定されている。2002年12月―2003年6月にボストン周辺にてAIPH症例が4例報告され、CDCおよびMassachusetts Department of Public Health(MDPH)により調査が行われた。症例は全て男児であり、生後48(28―77)日に発症、肺出血前2週間以内に全例上気道疾患を呈し、入院時に気道挿管および人工呼吸を要した。1例(症例A)にvon Willebrand病(vWD)の既往が報告されていたが、CDCにより2例(症例A、B)にてvWDを示す検査データが認められ、1例(症例D)は境界値を示し、さらに症例Bでは消化管出血を認めた。各症例の家庭環境調査では、症例A、B、Dの家庭において室内の埃や微粒子濃度の上昇が認められ、全家庭にてCladosporium、Penicillium種のカビの増殖が認められ、S.chartarumの胞子は2家庭で検出された。AIPHの発症を認めた場合には、vWDや他の出血性疾患の検査を行うべきである。
