Suspension of Rotavirus Vaccine After Reports of Intussusception - United States, 1999

腸重積報告後のロタウイルスワクチン接種の停止－アメリカ,1999年

1999年7月16日、認可されているロタウイルス4価ワクチン(RRV-TV)を接種した乳児15例における腸重積発症の報告を受け、CDCはRRV-TVの使用停止を勧告した。この報告では、RRV-TVによる有害事象を確認するためのサーベイランス活動、緊急対策、追跡調査結果を紹介する。Vaccine Adverse Event Reporting System(VAERS)に報告されたRRV-TV接種後の腸重積患者数は1998年12月18日―1999年6月2日には10例であったが、その後1999年7月6日までに15例に増加した。この増加は予想以上であったため、CDCは1999年7月16日のMMWR報告にてRRV-TVの初回接種後の3―7日以内に腸重積症のリスクが上昇することが確認された研究結果が明らかになるまでRRV-TVの使用を一時的に中止するよう勧告した。ロタウイルスワクチン接種プログラム停止から1ヶ月後の1999年8月15日以前に発症した腸重積患者は、1999年12月31日までに112例報告され、1999年7月16日以降にワクチン接種を受けた乳児および小児ではRRV-TV関連腸重積患者はみられなかった。VAERSの所見がより詳細な研究で確認された時点で製造業者は自主的にワクチンを回収し、Advisory Committee on Immunization Practices(ACIP)は1999年10月にRRV-TVの使用勧告を撤回した。系統的な調査、サーベイランス、科学的確認に基づいた迅速なリスク除去により、RRV-TVによる新たな腸重積患者の発生は避けられた。
