Breast- and Cervical- Cancer Screening Among Korean Women - Santa Clara County, California, 1994 and 2002

韓国人女性の乳癌および子宮頸癌スクリ―ニング―カリフォルニア州サンタクララ郡,1994および2002年

2000年、アメリカには約120万人の韓国人が居住しており、カリフォルニア州サンタクララ郡(人口2000年:170万人)では1.3%を占める。今回、サンタクララ郡で1994年8月―1995年2月、2002年2―6月における韓国人女性(2000年約12,000人)の乳癌および子宮頸癌のスクリ―ニング検査に関する調査が、カリフォルニア大学バ―クレ―校のCenter for Family and Community Health(CFCH)およびAsian Health Services(AHS)により実施された。調査は電話帳から韓国人姓を抽出して行った。質問は韓国語で行い、韓国語で回答しない場合は英語に変更して行った。適格例は1994年:501名(9.8%)、2002年:626名(5.8%)で、完全回答者は1994年:414名、2002年:458名であった。1994―2002年において、18―29歳の女性が占める割合は29.3%から18.1%に減少し、短大卒以上は61.7%から71.7%に増加、調査時前5年間に移住してきた人が9.9%から19.3%に増加しており、英語がほとんどまたは全く話せない人が61.0%から77.2%に増加していた。1994年の定期的健康診断受診率は40.5%、Pap検査実施率は65.0%(調査前3年以内の実施率は56.6%)、乳癌自己検査は66.3%が実施しており、23.6%は毎月行っていた。また、50歳以上の臨床的乳癌検査実施率は40.9%(調査前2年以内は29.2%)であり、マンモグラフィ―は43.3%(調査前2年以内は28.7%)であった。2002年には健康診断受診率は87.6%(調査前1年以内55.4%)、Pap検査実施率は74.8%(調査前3年以内は63.0%)、50歳以上の臨床的乳癌検査は55.4%(調査前2年以内は41.0%)、マンモグラフィ―は77.9%(調査前2年以内は58.9%)となった。定期的健康診断を受けている韓国人女性の検査率はカリフォルニア州の女性全体と比較した場合は低い。今後、文化に沿った教育的介入、より良い医療の利用や医療提供者のトレーニングがアジア系住民における乳癌や子宮頚癌のスクリーニングの向上に役立つであろう。
