関節炎および他のリウマチ性疾患(AORC)はアメリカにおける障害の主要原因である。AORCの影響は障害、外来管理、入院といった項目により評価される。アメリカでのAORCの直接費と間接費を概算するため、CDCは1997年Medical Expenditure Panel Survey(MEPS)のデータを分析した。1997年、18歳以上の国民3,840万人(14.2%)がICD-9-CMによるAORC患者定義に一致した症状を有していた。1997年のAORCの総費用は1,163億ドル(直接費511億ドル+間接費652億ドル)であり、アメリカの国内総生産の約1.4%であった。直接費511億ドルの内訳は、外来治療220億ドル、入院治療147億ドル、処方薬41億ドル、その他(在宅管理、医療装置など)65億ドルであった。18－64歳において、AORCによる間接費は652億ドルの逸失利益と概算された。州別のAORC関連総費用は1億6,300万(ワイオミング州)－113億ドル(カリフォルニア州)、直接費中央値は7億2,600万ドル(7,200万－50億ドル)、間接費中央値は9億2,600万ドル(9,100万－60億ドル)であった。より高価な治療(COX-2阻害薬、BRM、人工関節全置換術など)の実施増加などにより、今後医療費は大幅に増加すると予想される。州および地域の公衆衛生局はAORCにかかる費用の削減と患者のQOL改善に役立つさらなる自己管理プログラムを導入する必要がある。
