Yersinia enterocolitica Gastroenteritis Among Infants Exposed to Chitterlings - Chicago, Illinois, 2002

chitterling(食用のブタ小腸)に曝露した乳児におけるYersinia enterocolitica胃腸炎－イリノイ州シカゴ,2002年

2002年12月－2003年1月、Chicago Department of Public Health(CDPH)は、シカゴの1歳以下の乳児9例において10週間にわたり報告されたYersinia enterocolitica感染症について調査した。これら9例[年齢中央値8ヶ月(生後7週間－13ヶ月)]は2002年11－12月に発症した。9例中8例は黒人で、chitterlingsを調理したケアテイカーとの接触歴があり、うち1例はそれを食べた。残る1例は1歳のラテンアメリカ系乳児で、chitterlingsとの関連はみられなかった。曝露時間について情報が得られた7例は発症前の2週間以内[中央値4日間(1－12日間)]にchitterlingsとの直接あるいは間接的接触があった。6例のケアテイカーは場所は違うが同じチェーンの食料雑貨店3軒でchitterlingsを購入しており、2例は同じブランド品により曝露した。調理法はケアテイカーにより様々で、調理時間は2－12時間であった。発症に関連したchitterlingsまたはそのロットナンバーは入手できなかった。乳児2例から分離されたY.enterocoliticaの血清型はO:3であった。乳児1例の便培養からはchitterlings関連Y.enterocoliticaとともにSalmonella typhimuriumも分離された。全例とも回復し、症状は胃腸炎に限られていた。chitterlingsに曝露した乳児における腸感染症のリスクは持続しており、医療機関は特に冬季休暇中の黒人コミュニティにおいて胃腸炎の原因であるY.enterocoliticaに注意する必要がある。
