Cancer Mortality Among American Indians and Alaska Natives - United States, 1994-1998

アメリカンインディアンとアラスカ原住民における癌死亡率－アメリカ,1994～1998年

アメリカンインディアン/アラスカ原住民(AI/AN)の死因の第2位は癌である。AI/ANにおける癌死亡率を調査するため、Indian Health ServiceとCDCはNational Center for Health Statisticsを用い1994－1998年のアメリカ5地域(アラスカ、東部、北部平原地帯、南西部、太平洋沿岸部)におけるAI/ANの死亡データを分析した。AI/ANの癌死亡率(161.4/10万人)は、アメリカの全人種/民族における癌死亡率(205.5)に比べ低値であった。AI/ANにおける癌死亡率は男性(193.8)が女性(139.2)より高かった。AI/ANの癌死亡率には地域差がみられ、アラスカ(248.9)と北部平原地帯(291.7)で高かった。癌種別では、肺癌による死亡率がAI/AN(40.0)、全米(57.8)とも最も高かった。肺癌の死亡率にも地域差がみられ、アラスカ(78.1)と北部平原地帯(96.9)で高く、南西部(14.1)では低かった。AI/ANにおいて次に死亡率が高かった大腸癌(17.9)でも同様の地域差がみられた。子宮頚癌の死亡率はAI/AN(3.7)が全米(2.6)より高く、乳癌の死亡率はAI/AN(17.0)が全米(29.4)より低かった。東部においてAI/AN癌死例の80%はオクラホマ州居住者であった。癌予防および治療プログラムの計画や変更は地域差を考慮するべきであり、アラスカと北部平原地帯ではAI/ANを対象とした禁煙の奨励や大腸癌スクリーニングの普及を広める必要がある。
