Varicella-Related Deaths - United States, 2002

2002年、アメリカ衛生局はCDCに水痘関連死9例について報告した。カンザス州とイリノイ州で報告された水痘の成人1例と小児1例の臨床データを紹介する。症例1:2002年1月19日、37歳男性が激しい咳と息切れを訴えて救急外来を受診した。患者は3日前から皮疹が、4日前から発熱がみられていた。胸部X線所見などより水痘肺炎を疑い治療を開始したが、その後呼吸困難の進行、再発性気胸、心肺停止、無酸素性脳症、菌血症、左上肢深部静脈血栓症、昏睡といった合併症をきたし、3月9日に死亡した。受診前、患者の9歳の娘は水痘を発症しており(皮疹発現:1月3日)、5歳の娘も父親の発症から2日後に皮疹を発症した。3名とも水痘ワクチンを接種していなかった。症例2:1月14日、9歳女児が数日前からの水痘皮疹、左足への荷重不能、発熱を訴え緊急外来を受診した。水痘、敗血症および左下肢蜂巣炎の可能性があると診断され、ナフシリンの静注を受けた。その後呼吸困難が出現、心停止となり、初診から16時間後に死亡した。患者は放課後の学童保育施設と学校で計3例の水痘患児に曝露していた。これら水痘患児は4名ともワクチン接種を受けていなかった。このように水痘は深刻となる可能性がある。今後、学童保育施設や学校での水痘ワクチン接種を確実にするなどの水痘予防戦略が重要と考える。また水痘関連死サーベイランスの改善が必要性である。
