Hepatitis C Virus Transmission from an Antibody-Negative Organ and Tissue Donor - United States, 2000-2002

抗体陰性のドナー器官および組織からのC型肝炎ウイルス伝播－アメリカ,2000～2002年

Oregon Department of Human Services(DHS)とCDCによる移植ドナーからのC型肝炎伝播についての調査の予備的結果。2002年6月、DHSは膝蓋腱の同種移植から約6週間後に急性C型肝炎を発症した患者の報告を受けた。この患者のドナーの死亡時(2000年10月)、その血清からはC型肝炎ウイルス(HCV)抗体は検出されなかった。2002年7月、凍結保存してあった血清を再検査したところ、ドナーはHCV抗体陰性であったがHCV-RNA陽性(遺伝子型1a)であることが明らかになった。ドナーから摘出された91片の器官および組織のうち44片が、2000年10月－2002年7月に40例のレシピエントに移植されていた(器官6例、組織32例、角膜2例)。全組織は抗菌処理、骨移植片はガンマ線照射を行った。40例中8例でHCV(遺伝子型1a)が確認された。器官レシピエントのうち移植後に血清が入手できた3例では全例が感染していた。組織レシピエントでは、移植前にHCV感染者と判明した3例と検査結果を入手できなかった2例を除く残り27例のうち5例(伏在静脈1例、腱1例、骨と腱3例)で感染の可能性を認めた。他の1例では移植後に遺伝子型3aのHCV感染が認められた。皮膚と照射骨を移植したレシピエント(それぞれ2例、16例)では感染は認められなかった。角膜を移植したレシピエントの1例は移植前の感染例であり、残る1例は抗HCV陰性であるが3月27日現在HCV-RNAの検査は行っていない。抗HCV陰性のドナー組織によるHCV伝播はまれと思われるが、そのようなドナーからの移植の頻度とレシピエントへのHCV伝播リスクを検討することは感染予防処置の正当性を評価する上で重要である。
