Trends in Tuberculosis Morbidity - United States, 1992-2002

米国における2002年の全国結核(TB)サーベイランスシステムのデータと過去10年間のTB罹患率の傾向についての概要。2002年にCDCに報告されたTB患者数は15,078例で、2001年に比べ5.7%、TB再流行のピークであった1992年に比べ43.5%減少した。2002年のTB罹患率は米国で1953年に報告が開始されて以来最低となった。1992年以降、全ての年齢群、人種/民族集団、地域でTB罹患率の減少がみられている。しかし2002年の罹患率である10万人あたり5.2人は、全国的TB根絶計画の一部として設定された2000年暫定目標である10万人あたり3.5人(2010年までの目標:10万人あたり1人未満)よりも多い。また、TB発症数は全体的に減少しているものの、国外出生者および米国で出生した非ラテンアメリカ系黒人集団における罹患率と、その他の米国住民における罹患率に不均衡がみられ、現在、全TB患者のうち国外出生者(7,544例)と米国で出生した非ラテンアメリカ系黒人(3,350例)が約3/4を占めている。米国における今後のTB根絶への進展は、潜在性TB感染者である国外出生者へのサービス提供プログラム、全体的なTB疾患の負荷を軽減させるための努力、米国で出生した非ラテンアメリカ系黒人集団におけるTBコントロールの強化にかかっている。
