検査室で感染した西ナイルウイルス感染症－米国，2002年

経皮的注入による伝播以外、危険因子がみられなかった検査室職員2名における西ナイルウイルス(WNV)感染症についての報告。1例は、2002年8月にバイオセーフティーレベル2(BSL-2)の条件下にて鳥の脳(WNV RNA陽性)を処理中、メスで親指に裂傷を負い、その4日後に頭痛、筋肉痛、倦怠感の他、知覚不全やリンパ節腫脹などが発現した。さらに2日後には顔面に斑丘疹が出現して四肢にまで広がった。他の1例は2002年10月にBSL-3の条件下にて WNV感染マウス脳を処理中、汚染された針で指を刺し、その3日後に上気道感染症状が発現した。これら2例はともに最近蚊に刺されたことがなく、長期間の屋外活動や輸血の経験もなかった。両例は血清学的検査にて急性WNV感染のエビデンスを認め、前例は発症13、21日目、後例は発症10日目に採取した血清検体でWNV-特異的IgM抗体陽性を示した。2例とも症状は軽度で自己限定的であった。検査室職員は職業上WNVの感染リスクを有する。WNV感染が判明、あるいはその疑いのある組織を取り扱う場合は曝露リスクを最小限にとどめることに注意すべきであり、仕事が原因と考えられる負傷や疾患は直ちにに報告することが重要であること念頭に置くべきである。
