Iron Deficiency - United States, 1999-2000

米国では1990年代早期、歩き始めの幼児と妊娠可能年齢の女性で鉄欠乏症の有病率が著明に高いことが明らかになった。2010年までの国民健康目標の一つは、これら罹患しやすい集団における鉄欠乏症有病率を3－4%に減少させることである。米国民における生化学的鉄測定値(ヘモグロビン、血清フェリチン、トランスフェリン飽和度、遊離赤血球プロトポルフィリン)の特徴を明らかにするため、CDCは1999－2000年National Health and Nutrition Examination Survey(NHANES 1999－2000)のデータを用いて鉄欠乏症と鉄欠乏性貧血の有病率を算出し、その値を1988－1994年に実施されたNHANES 3のデータと比較した。結果、鉄欠乏症有病率は1－2歳の幼児(7%)と12－49歳の女性(9－16%)で最大であった。また非ラテンアメリカ系黒人およびメキシコ系アメリカ人女性の鉄欠乏症有病率(19－22%)は、非ラテンアメリカ系白人女性(10%)に比し約2倍高かった。NHANES 1999－2000とNHANES 3における鉄欠乏症有病率はほとんどの年齢群および性別群で同様であったが、12－69歳の男性と50－69歳の女性ではNHANES 1999－2000における有病率がNHANES 3に比しかなり高値を示した。鉄欠乏症有病率が最も高かった集団における鉄欠乏性貧血の有病率は5%未満であり、NHANES 1999－2000とNHANES 3で差はみられなかった。鉄欠乏症の予防のため、罹患しやすい集団には鉄分の豊富な食物や母乳栄養の摂取、あるいは鉄分を強化した乳児用人工乳の使用を奨励するべきである。
