州別脳卒中死亡率および死亡場所の分布－米国，1999年

1999年の米国における全国および州別の脳卒中死亡率を報告する。1999年には米国居住者のうち167366例が脳卒中関連死を来しており、年齢調整死亡率(10万人あたり)は63.4であった。年齢別にみると、85歳以上が最も多く(40.1%)、次が75歳から84歳(34.3%)であった。年齢調整死亡率は人種別では黒人(225.2)、白人(166.7)の順に高く、また男性(62.4)は女性(60.5)よりも若干高かった。虚血性発作は全脳卒中死の68.3%を占めた。これら脳卒中関連死亡例のうち、救急部(ED)への搬送前に死亡したのは79,633例(47.6%)、ED到着時に死亡していたのは926例(0.7%)、EDで死亡したのは5,519例(3.3%)、入院後死亡したのは80,369例(48.0%)であった。脳卒中死亡率と搬送前死亡率は州によって異なり、年齢調整死亡率は33.0(ニューハンプシャー)－83.8(サウスカロライナ)、搬送前死亡率は23.3%(コロンビア特別区)－67.3%(オレゴン)の範囲であった。搬送前死亡率が60%以上であった州は、コロラド(60.0%)、ウィスコンシン(60.7%)、ユタ(60.7%)、ミネソタ(62.1%)、アイダホ(64.0%)、ワシントン(64.4%)、バーモント(67.2%)、オレゴン(67.3%)であった。また州別のED到着時脳卒中関連死亡率は0%から4.6%、EDでの死亡率は0.8%から8.3%であった。一般人を対象として脳卒中の徴候や症状、ED搬送の重要性などについて教育し、その発症を予防することが、脳卒中関連死および障害を減少させる重要戦略である。
