肺炎連鎖球菌に対する感受性試験実施についての評価－米国，2000年2月

肺炎連鎖球菌は米国における市中肺炎や中耳炎、髄膜炎の主因であり、抗菌薬感受性試験の結果は治療法の決定や耐性パターン発現のモニタリングに重要である。肺炎球菌感受性試験の適切な方法は、National Committee for Clinical Laboratory Standards(NCCLS)により勧告されている。2000年2月、CDCは米国内の臨床検査室を対象とし、滅菌部位由来分離菌における肺炎連鎖球菌感受性試験実施について調査を行った。回答が得られた547の検査室中452(83%)の検査室が肺炎球菌分離菌の感受性試験を行っており、このうち353(78%)の検査室は自分の検査室で一部の試験を行っていた。353の検査室のうち188(53%)は、自分の検査室で滅菌部位由来分離菌についてNCCLSにより勧告された血液およびCSF分離菌用のオキサシリンスクリーニングを行っていた。また自分の検査室で抗菌薬のMIC測定あるいはディスク拡散法を行っていた250(71%)の検査室のうち、190(76%)はNCCLSにより勧告された3種類の抗菌薬(ペニシリン、セフォタキシム/セフトリアキソン、バンコマイシン)に対する耐性の検討を行っていた。さらに250の検査室のうち、137(55%)はNCCLSの勧告通り感受性試験結果の解釈と正確なMIC値を医師に報告していた。肺炎球菌の抗菌薬耐性は悪化し続けているため、臨床検査室は耐性パターンの発現に注意し、NCCLSの勧告に従い抗菌薬感受性についての正確な情報を医師に提供するべきである。
