2000年、米国では仮総計で86例の麻疹確定症例がCDCに報告された。86例中26例(30%)は米国以外からの輸入感染例であった。また60例の国内感染例のうち18例は輸入感染に関連しており、9例は輸入ウイルスによるものであり、33例は感染源不明であった。麻疹は10か国から輸入されており、そのうち日本からが最も多く(7例)、続いて韓国とエチオピア(各4例)であった。症例は20州で報告されており、そのうちニューヨーク州(23例)、カリフォルニア州(19例)、ネバダ州(7例)での報告例が57%を占めた。86例のうち68例(79%)は1－26週、18例(21%)は27－52週に発症した。また10例(12%)は1歳未満、27例(31%)は1－4歳、17例(20%)は5－19歳、20例(23%)は20－34歳、12例(14%)は35歳以上であった。86例中23例(27%)は麻疹ワクチン接種歴の記録があったが、40例(46%)はワクチン接種を受けておらず(このうち9例は1歳未満)、23例(27%)はワクチン接種歴が不明であった。2000年には麻疹アウトブレイクが9州で10回起こり、86例中48例(56%)はこのアウトブレイクによる症例であった。10回のアウトブレイクのうち5回は疫学的に輸入感染例が関与していた。2000年の麻疹報告症例数は、過去2年間の報告数から14%減少した。依然として風土病性麻疹は報告されていないが、国内伝播をもたらす可能性のある米国以外からの輸入感染リスクは持続している。輸入感染例からの国内伝播拡大を制限するためには、高いワクチン接種率の維持が重要である。
