Barriers to Dietary Control Among Pregnant Women with Phenylketonuria - United States, 1998-2000

フェニルケトン尿症妊婦における食事管理の障害－米国，1998～2000年

2000年秋、CDCは1998年から2000年に妊娠(index妊娠)した22歳から38歳のフェニルケトン尿症(PKU)女性24例を対象として、食事管理や妊娠転帰についてのインタビューによる調査を行った。結果、24回のindex妊娠のうち18回(75%)は生産児を得、11回の妊娠は計画的であった。妊娠前の治療食使用率は、計画妊娠11例で非計画妊娠女性よりも高かった。1例は成年期中、23例は6年間から24年間食事制限をしていた。22例は妊娠前あるいは妊娠中に治療食を再開し、8例は妊娠前、11例は妊娠後(妊娠10週以降)に代謝疾患専門医を訪ねていた。22例の診療記録によると、12例は妊娠10週以前に血中フェニルアラニン(phe)濃度がコントロールされていた。　症例1：青年期早期に治療食を中止し、代謝疾患専門医にも行っていなかった21歳の女性は小頭症および発育遅滞を有した児を出産した。症例2：経済的理由から成人期早期に治療食を中止し、近くに代謝疾患専門医もなかった21歳女性も、小頭症の児を出産した。症例3：成人期にもPKU食を維持し、代謝疾患専門医に通院していた27歳女性は計画妊娠により健常児を出産した。　PKU女性が妊娠前あるいは妊娠中に治療食を摂取しておらず妊娠中の母体血中phe濃度が高い場合には、出生児が精神遅滞や小頭症を有するリスクが非常に高い。したがって、PKU女性では妊娠前および妊娠中の食事管理維持に対する障害を克服することが重要であり、障害克服のための効果的なアプローチについて検討する必要がある。
