2型ポリオウイルスの封じ込めに向けた進展 ― 全世界、2017年
本レポートは、2015年の世界保健総会にて承認された世界行動計画(GAPIII)に基づく2型ポリオウイルス(PV2)の封じ込めに向けた進展をまとめたものである。野生株ポリオウイルス(WPV)のうち、1型WPVの常在国は現在3カ国(アフガニスタン、ナイジェリア、パキスタン)にまで減少、症例数も2015年に74例、2016年に37例、2017年6月半ばまでに6件と漸減している。3型WPVは2012年11月にナイジェリアで発生した症例が最後となっている。2型WPV2は1999年の検出を最後に、2015年9月に根絶宣言された。しかし、ワクチン株由来伝播型2型ポリオウイルス(cVDPD2)の流行が発生したことから、2016年4~5月、WHOはワクチン株1、2、3型を含む3価の経口生ポリオワクチン(OPV)を使用停止し、2型ワクチン株を除いた2価OPVへの転換を全世界で実施した。根絶後にポリオウイルス取り扱い施設からPV2が流出するリスクを最小化するため、GAPIIIに則り、WHOはPV2の封じ込めを開始した。封じ込めの第1段階として、WHOの全加盟国および地域は、自国の検査機関およびワクチン製造施設におけるPV2の感染性材料の保有状況調査を完了し、PV2野生株およびワクチン株の廃棄または保有継続を決定し、WHOの認証を受けた。Global Polio Laboratory Network(GPLN)の全146検査機関は、2016年7月31日までにPV2を疑われる全検体をpoliovirus-essential facilityに移し、保管、施錠を完了した。一方、パキスタン、シリア、コンゴ共和国、ナイジェリアおよびチャド湖周辺地域を含む計8カ国では、cVDPV2のアウトブレイクに備えて単価の2型OPVが承認されており、cVDPV2およびワクチン株の封じ込めには至っていない。第2段階として、PV2の保有継続を決定した30カ国は、2017年6月半ば時点でGPLNに属する146検査機関のうち21機関(14.4%)を含む計86施設をポリオワクチンの製造、疾病診断および研究拠点であるpoliovirus-essential facilityに認定した。30カ国のうち18カ国はpoliovirus-essential facilityにおけるバイオリスク管理を監督、認証する封じ込め責任監督機関を設置し、WHOに報告した。
