Tobacco Use Among Middle and High School Students — United States, 2011–2016
ミドルおよびハイスクール生の喫煙 ― アメリカ、2011~2016年
2011~2016年のNational Youth Tobacco Surveysのデータから、ミドル(6~8年生)およびハイスクール(9~12年生)における7種類のたばこ製品[電子たばこ、紙巻きたばこ、葉巻、無煙たばこ、水たばこ、パイプたばこ(刻みたばこ)、ビディ]の喫煙状況について分析が行われた。サンプル数(回答率)は2011年:18,866(72.7%)、2012年:24,658(73.6%)、2013年:18,406(67.8%)、2014年:22,007(73.3%)、2015年:17,711(63.4%)、2016年:20,675(71.6%)であり、現喫煙者は過去30日以内に1日以上使用した者とした。2016年、ハイスクールの生徒は20.2%(推定305万人)が現喫煙者であり、9.6%(144万人、現喫煙者の47.2%)は2種類以上を使用していた。製品別では電子たばこが最も多く(現喫煙者の11.3%)、紙巻きたばこ(8.0%)、葉巻(7.7%)、無煙たばこ(5.8%)、水たばこ(4.8%)、パイプたばこ(1.4%)、ビディ(0.5%)の順であり、男女別ではすべての製品および2種類以上の現喫煙者いずれも男子が多かった。また、非ヒスパニック系白人およびヒスパニック系は電子たばこがもっとも多く、非ヒスパニック系黒人は葉巻がもっとも多かった。ミドルスクールの生徒は7.2%(推定85万人)が現喫煙者であり、3.1%(36万人、現喫煙者の42.4%)が2種類以上を使用していた。製品別では電子たばこ(4.3%)、紙巻きたばこ(2.2%)、葉巻(2.2%)、無煙たばこ(2.2%)、水たばこ(2.0%)、パイプたばこ(0.7%)、ビディ(0.3%)の順であり、男女別の現喫煙率は男子:8.3%、女子:5.9%であった。ハイスクールにおける現喫煙率は2011~2016年にかけて有意な変動は認めず(2011年24.2%、2016年20.2%)、可燃性の製品(紙巻きたばこ、葉巻、水たばこ、パイプたばこ、ビディ)の現喫煙率は21.8%から13.8%、2種類以上の現喫煙率は12.0%から9.6%へ低下した。製品別では電子たばこ(1.5%から11.3%)、水たばこ(4.1%から4.8%)は増加、紙巻きたばこ(15.8%から8.0%)、葉巻(11.6%から7.7%)および無煙たばこ(7.9%から5.8%)は直線的に低下、パイプたばこ(4.0%から1.4%)、ビディ(2.0%から0.5%)は非直線的に低下した。ミドルスクールにおける現喫煙率は可燃性の製品(6.4%から4.3%)、紙巻きたばこ(4.3%から2.2%)、葉巻(3.5%から2.2%)およびパイプたばこ(2.2%から0.7%)は直線的に低下、電子たばこ(0.6%から4.3%)および水たばこ(1.0%から2.0%)は増加していた。2015年との比較ではミドルスクールでは有意な変動は認めず、ハイスクールでは全体の現喫煙率が25.3%から20.2%と有意に低下し、可燃性の製品、2種類以上の製品および水たばこの現喫煙率が有意に低下し、電子たばこの喫煙率は有意に増加していた。以上、包括的および継続的な禁煙戦略によりアメリカの若者において喫煙率が低下していることが示された。
