Trends in Repeat Births and Use of Postpartum Contraception Among Teens — United States, 2004–2015

10代における繰り返しの出産と出産後の避妊の傾向 ― アメリカ、2004～2015年

10代における繰り返しの出産(20歳以前の2回以上の妊娠、出産)は、母親の教育や就職の機会を奪い、また早産や低出生児体重の危険性も高い。1991～2015年の間に、アメリカにおける15～19歳の出産率は女性1,000人あたり61.8件から22.3件に低下したが、10代での繰り返しの出産は未だに多い。CDCは、National Vital Statistics System natality filesの2004年および2015年のデータおよびPregnancy Risk Assessment Monitoring Systemの2004～2013年のデータを用い、10代での繰り返しの出産と産後の避妊について解析した。2015年、15～19歳の女性における出産413,144件のうち、繰り返しの出産は38,324件(16.7％)であった。10代での繰り返しの出産率は、ヒスパニック系(18.7％)、非ヒスパニック系黒人(17.9％)、非ヒスパニック系白人(14.3％)で高かった。州別では、バーモント州の10.6％からワシントンDCの21.4％と幅があった。全体の10代の繰り返しの出産件数は、2004年の82,997件から2015年の38,324件へ53.8％減少し、繰り返しの出産率も20.1％から16.7％へ16.9％低下した。2004年から2015年にかけ、10代の繰り返しの出産率は20％超の低下を示した12州を含む35州にて有意に低下し、上昇した州はなかった。2013年、生産の産後間もない10代の女性のうち82.8％が産後に避妊具を使用し、うち26.9％は高効果、40.2％は中等度効果、15.7％は低効果の避妊法を使用と回答した。高効果避妊法の使用率はニューヨーク市の11.4％からコロラド州の51.5％まで、非避妊率はバーモント州の4.9％からニュージャージー州の33.8％まで幅があった。2004～2013年、10代の産後の避妊についてデータを継続的に収集していた5州において、避妊具の使用率はほぼ一定であったが、高効果避妊法(子宮内避妊装置、インプラント)の使用率は2004～2005年の5.3％から2012～2013年の25.3％へ有意に増加した。その一方、2013年には約3分の1の女性が低効果避妊法を使用(15.7％)または避妊していない(17.2％)と回答した。10代での繰り返しの出産を予防するには、効果の高い避妊法の使用を促進する包括的な対策が必要である。
