HIV Services Provided by STD Programs in State and Local Health Departments — United States, 2013–2014

州および地域の保健局においてSTDプログラムにより提供されるHIV医療サービス－アメリカ、2013年～2014年

アメリカにおけるHIV感染症の発症率は性感染症(STD)患者にて高く、また、HIV感染症患者ではSTDの発症率が高い。STD感染はHIV感染リスクを増加するため、STDの治療によりハイリスク例におけるHIV感染の拡大を抑制しうると考えられ、アメリカ保健福祉省によるSTDプログラムはHIVプログラムと一部またはすべてが統合されている。今回、各州および地域の保健局によるSTDプログラムによるHIV医療サービスの提供について、地域の保健局311施設(地域)と州または直接管轄地域の保健局56施設(州)のうち、回答が得られたそれぞれ148施設、33施設を対象に分析した。州の保健局では、HIVに関する医療従事者の訪問プログラムを94.1％の施設が実施していたが、地域の保健局では46.7％と有意差を認めた。他に、ハイリスク例を対象とした予防活動(州：92.3％、地域：58.4％)、STD患者とHIVデータとの相互的分析(同73.1％、16.1％)などのプログラムにも有意差を認めた。STD接触者に対するHIV検査は州が39.4％、地域が25.0％にて実施し、地域の保健局に関しては管轄地域が小さい保健局での検査率は11.4％と低く(中程度および大きい地域ではそれぞれ37.8％、39.0％)、医療従事者の訪問も管轄地域が小さい保健局にて26.0％(中程度：59.0％、大きい地域：95.6％)と低かった。以上、STDプログラムによるHIV医療サービスの提供は州の保健局と地域の保健局にて相違が認められ、改善の余地が示唆された。
