Baseline Prevalence of Birth Defects Associated with Congenital Zika Virus
先天性ジカウイルス感染症に関連した先天性障害のベースラインの有病率 ― マサチューセッツ州、ノースカロライナ州、ジョージア州アトランタ、2013~2014年
妊娠中のジカウイルス感染は重症の脳異常の原因となるが、この有害な転帰の全容は明らかではない。CDCはジカウイルス感染が原因の先天性障害の影響を調査するために、2016年にジカウイルス感染と関連する可能性がある先天性障害のために確立した症例定義を、ジカウイルスがWHOのアメリカ地域への流入前の2013~2014年にマサチューセッツ州、ノースカロライナ州、ジョージア州アトランタの3つの地域から収集した人口をベースとした先天性障害の監視データにレトロスペクティブに適用した。これらの3つの地域からのデータには、症例定義に合致した1つ以上の先天性障害を持つ乳児または胎児747例が含まれ、ジカウイルス流入前の有病率は生児1000人あたり2.86人であった。障害の頻度(1000人あたり)は脳異常および小頭症最も高く(1.50人)、次いで神経管欠損およびその他の早期脳形成異常(0.88人)、脳異常の言及のない目の異常(0.31人)、脳または目の異常の言及のない中枢神経系機能障害(0.17人)であった。一方、2016年1月15日から9月22日では、Zika Pregnancy Registryには妊娠中のジカウイルス感染の可能性の臨床検査上のエビデンスのある母親から出生し、妊娠期間を完遂した442例のうち、26例の乳児および胎児に同じ障害が報告された(1000人あたり58.8人)。確認手法が異なるが、ジカウイルス流入後では、先天性障害を胎児および乳児の有病率は、ジカウイルス流入前の年の妊婦間の先天性障害の有病率よりも約20倍増加した。これらの結果は、ジカウイルス感染に関連した可能性のある先天性障害についてのデータを解釈するために、人口をベースとした監視の重要性を示している。
