CDC Grand Rounds：慢性疲労症候群 ― 研究の進歩と臨床教育

慢性疲労症候群(CFS)は1980年代後半にCDCが名付け、他に筋痛性脳脊髄炎(ME)、ME/CFS、慢性疲労免疫機能不全、最近では全身性労作不耐性疾患とも呼ばれる。ここではME/CFSを使用する。患者は女性が多く(男性の3～4倍)、年齢は40～50歳代に多い。生産性の損失は90～370億ドル、直接医療費は90～140億ドルであり、医療費の約1/4は患者またはその家族が支払っている。罹病期間は多くの症例が5年間以上であり、約半数の患者が治療を必要としている。臨床評価は既往歴、心理社会歴、身体検査、精神健康評価、生化学的検査により行う。原因は明らかではないが、インフルエンザ様疾患の急性発症がトリガーである症例が多く、エプスタイン・バーウイルス、ロスリバーウイルス、コクシエラ(Q熱)またはジアルジア症などの感染症後の発症も10％ほど認められ、感染症の重症度によりその後の疾患の重症度が予測される。また、健常人と比較した場合、ME/CFS患者はストレスが大きく、メタボリック症候群の患者が多い。ME/CFSの治療法は確立していないが、最初に認められる症状である睡眠障害に対しては睡眠または疼痛治療の専門家による指導療法が有効である。非薬物療法としてエプソムソルト浴、マッサージ、鍼治療、そしてもっとも重要な活動管理などがある。最近、Institute of Medicine (IOM)およびNational Institute of Health (NIH)は、ME/CFSは精神疾患ではないがうつ病を引き起こす可能性があるとし、IOMは新しい症例基準を発表し、NIHは2015年9月より研究プロトコール作成のためのプログラムを展開している。また、CDCはME/CFSに関する情報を医療関係者に提供し、2012および2013年にはMedscapeと提携してプライマリケア医を対象とした円卓会議を開催している。最近は関係者と協力し、教育資料の作成を開始している。
