全米公立医療機関認証による影響の評価 ― アメリカ、2016年

アメリカには約2,500の公立医療機関が存在する。2011年、非営利の公立医療機関認証委員会(PHAB)は、これら医療機関の質とパフォーマンスの向上を目的として、自発的な医療機関認証プログラムを立ち上げた。このプログラムでは、認証を受けたい医療機関は「公共医療サービスにおける10の重点項目」について自己評価を行い、不十分な点について対策を立て、住民の健康評価、住民の健康向上プラン、医療機関の行動計画などの書類を作成する。ボランティアの訪問審査官がこれらの書類を精査し、対策について評価を行う。PHABの認証委員は訪問審査官の報告を受け、医療機関の認証の可否を判定する。2016年5月現在、134の州および地方公立医療機関がPHABより5年間の認証を取得し、176以上の機関が認証前プロセスを実施している。42州およびワシントンDCには、認証を取得した医療機関が1件以上あり、これにより全米人口の約半数が認証医療機関を受診できる状況になっている。2013年、PHABは社会科学系研究機関であるシカゴ大学NORCに依頼し、この認証プログラムの改善点と医療機関に与える影響の検証を行った。2013年10月～2016年1月、NORCは認証を取得して1年以上経過した60医療機関を対象に調査を実施、52機関(87％)から解答を得た。多くの医療機関が、認証プログラムの実施により、質とパフォーマンスの向上が図られた(98％)、長所と弱点の把握につながった(96％)、説明責任と透明性の向上につながった(92％)、病院首脳部の管理プロセスが改善された(90％)、と解答した。
