Elimination of Mother-to-Child Transmission of HIV — Thailand

タイでは1990年代、ヒト免疫不全ウイルス(HIV)が流行し、妊婦における罹患率は2.0％、母子感染(MTCT)率は20％を超えていたが、2016年6月、アジアで初めてWHOが目標とするMTCT撲滅に達した。タイで初めて妊婦がHIVに感染したのは1988年であり、その後増加していたが、1996年、ACTG 076試験[HIV感染妊婦を対象としたジドブジン(AZT)の経口投与と静注投与による効果を比較した第III相試験]の後、Thailand Ministry of Public Health (MOPH)とシリラート病院はCDCと共同し、短期間の経口AZTの試験を開始、この試験でMTCTの50％抑制を得た。1996年、ソアンサワリ王妃がMTCT予防のため赤十字に資金を提供、1997～1999年、MOPHはタイ北東部および北部にてMTCT予防のパイロットプロジェクトを開始し、妊婦を対象としたHIV検査、AZTの予防投与、さらにMTCTのモニタリングシステムを開始した。2000年にはMOPHの健康局(DOH)がMTCT予防策(妊婦のHIV検査、抗レトロウイルス治療および母親がHIV陽性の場合の乳児に対する授乳)に関するガイドラインを発表した。抗レトロウイルス治療は2000年、HIV陽性妊婦に対し、妊娠34週からAZTを投与し、生まれた乳児に4週間AZTを投与するとされていたが、2004年にはネビラピン単回投与が追加され(WHOオプションA)、2010年には高活性抗レトロウイルス治療(オプションB)として妊婦のCD4細胞数に基づき治療を継続、2014年にはCD4細胞数にかかわらず、高活性抗レトロウイルス治療を一生継続する(オプションB+)と進歩した。乳児の検査ガイドラインも進歩し、2000～2006年は抗体検査により生後12、18カ月にて診断されていたが、2007年には生後1～2カ月および2～4カ月にてHIV DNA PCR検査を実施、2014年には感染リスクの高い乳児に対して、生後4週未満より高活性抗レトロウイルス治療が開始される。また、MTCTモニタリングシステム(Perinatal HIV Intervention Monitoring System)により妊婦のHIV検査率は1998年：61.9％、2001年：92.9％、2015年：99.6％、MTCT予防のための抗レトロウイルス治療の実施率は1998年：64.6％、2001年：71.4％、2015年：95.6％となり、MTCT目標値(それぞれ95％、90％)に到達した。これらの予防策の実施により、MTCTの発症率は1994年：24.2％から2003年：10.2％、2015年：1.9％と大きく低下し、これらの成果は他の国のモデルとなっている。
