Cigarette Smoking Among Urban American Indian Adults — Hennepin and Ramsey Counties, Minnesota, 2011

都市のアメリカインディアン成人における喫煙 ― ミネソタ州ヘネピン郡およびラムゼイ郡、2011年

2013年のアメリカインディアンにおける喫煙率は推定36.5％とアメリカにおけるすべての人種/民族の中で最も高かった。アメリカインディアンの中でも喫煙には文化的および地理的な多様性が認められ、喫煙率は北西部州で低く、中西部北部州/北部平原州で高いが、インディアン居留地に居住しない都市のアメリカインディアンにおける喫煙状況については明らかでない。ミネアポリス市およびセントポール市を擁するヘネピン郡およびラムゼイ郡にはミネソタ州のアメリカインディアンの30％が居住することから、両郡にてアメリカインディアン成人の喫煙調査を実施した。適当な対象者リストが存在しないことから、調査は特定の回答者が次の回答者を紹介するrespondent-driven sampling法にて行い、2011年3～5月に計964名の回答を得、そのうち940名の回答が有効であった。喫煙は、儀式的喫煙と非儀式的喫煙を区別した。回答者の多くがオジブワ族(65％)、ダコタ/ラコタ族(20％)、その他(2％)であった。現在喫煙者(生涯に100本以上喫煙し、現在毎日または時々喫煙)、過去喫煙者(生涯に100本以上喫煙したが、現在喫煙していない)および非喫煙者(喫煙したことはない)はそれぞれ59.3％、18.5％および22.1％であった。現在喫煙者の割合は、女性(55.8％)が男性(63.7％)より低く、年齢層別では25～44歳(72.4％)で最も高かった。1日の喫煙本数は5本以下(39.4％)が最も多いが、70.5％が直前1カ月以内に20日以上喫煙していた。タバコの種類は、フィルター付きタバコ(48.3％)およびメントール(42.1％)が多かった。喫煙経験者の23.8％が禁煙しており、現在喫煙者の3分の2 (67.6％)が禁煙を望み、約半数(57.2％)は1年以内に禁煙を試みていた。禁煙方法は多く(42.9％)がニコチン置換療法を希望し、電話による禁煙サポート(14.7％)は好まれず、個別(36.9％)またはグループ(25.4％)での禁煙サポートを希望した。被雇用者の多く(97％)はインディアン居留地以外で働いており、自宅(41.5％)、車内(64.3％)、職場(40.8％)における受動喫煙率は高く、いずれもミネソタ州全体の居住者を対象にした調査(2010年)よりも高かった。都市部のアメリカインディアンにおける喫煙および受動喫煙率は高く、文化的に適切なアプローチでの禁煙推進が必要である。
