Zika Virus Infection Among U.S. Pregnant Travelers — August 2015–February 2016

アメリカの妊婦渡航者におけるジカウイルス感染症 ― 2015年8月～2016年2月

妊娠中にジカウイルスに感染した母親から生まれた新生児の小頭症および他の有害な妊娠転帰が報告され、2016年1月15日、CDCはジカウイルス感染の流行地域への妊婦の渡航に対する注意を発表し、さらに1月19日にはアメリカの医療関係者に対し、流行地域へ渡航した妊婦のケアに関する暫定的ガイドラインを発行した。2月17日の時点でCDCに報告されたジカウイルス感染が検査で確診された妊婦の渡航者は9例であり、さらに10例が調査中である。入院および死亡例は報告されておらず、9例の妊娠転帰は妊娠早期流産：2例、選択的中絶：2例、出産：3例(健常児：2例、重度小頭症：1例)、妊娠中：2例(18週、34週)であり、渡航した地域はアメリカ領サモア、ブラジル、エルサルバドル、グアテマラ、ハイチ、ホンジュラス、メキシコ、プエルトリコおよびサモアであった(2月18日の時点で感染の流行は32の国と地域にて認められている)。このうちの4例について報告する。症例A)2016年1月、30代の女性が妊娠6～7週にて発熱、発疹、関節痛、筋肉痛および倦怠感を来した。妊娠のおよそ5週目に流行地域へ渡航しており、血清学的検査にて感染が確認された。妊娠8週にて早期流産となり掻爬術が行われた。症例B)2016年1月、30代の妊婦がジカウイルス感染の検査を受けた。この女性は妊娠11～12週にて流行地域へ旅行し、帰国した翌日に発熱、眼痛および筋肉痛、次の日に発疹を来した。妊娠20週にて胎児MRI検査により重度の脳萎縮を認めたため、選択的中絶を行った。症例C)2015年末、30代の女性が妊娠39週にて出産したが、出生時の頭囲が27cmと重度の小頭症であった。この女性は妊娠12週までブラジルに住んでおり、妊娠7～8週に発熱、発疹、関節痛および頭痛を来しており、血清学的検査によりジカウイルス感染が確認された。症例D)30代の妊婦が妊娠15週にて流行地域へ渡航し、発熱、発疹、関節痛および頭痛を来し、血清学的検査にてジカウイルス感染が確認されたが、妊娠40週にて健常児を出産した(頭囲：34.5cm)。
