Advisory Committee on Immunization Practices(ACIP)は、2014年の季節性インフルエンザワクチンの使用に関する推奨を改訂し、2015-2016年シーズンの最新情報には、1)アメリカの季節性インフルエンザワクチンの抗原組成、2)2015-2016年シーズンの利用が期待できるインフルエンザワクチン製品に関する情報、3)生後6カ月～8歳児に対する適切な投与回数を決定するためのアルゴリズム更新、4)弱毒化インフルエンザ生ワクチン(LAIV)および不活化インフルエンザワクチン(IIV)のどちらも利用可能な場合の使用の推奨(2014-2015年の2～8歳の健康な小児に対するLAIVの優先的な推奨の取り消しを含む)が記載されている。年に1回の定期的なインフルエンザワクチン接種は、生後6カ月以上の禁忌のないすべての年齢の人に推奨されており、インフルエンザの活動性が高くなる前に接種することが好ましい。2015-2016年シーズンに向けてアメリカで認可された3価インフルエンザワクチンには、A/California/7/2009 (H1N1)-likeウイルス、A/Switzerland/9715293/2013 (H3N2)-likeウイルス、 B/Phuket/3073/2013-like (Yamagata lineage)ウイルス由来のヘマグルチオンが含まれる予定であり、4価ワクチンではこれらのウイルスに、B/Brisbane/60/2008-like (Victoria lineage)ウイルスが追加される。2015-2016年シーズンでは、さまざまなインフルエンザワクチン製品が利用可能になると予測されている。生後6カ月～8歳児では反応を最適にするために、ワクチン接種の初めてのシーズンでは2回の接種(最低4週間間隔)が必要である。2009年から主要なインフルエンザA(H1N1)としてインフルエンザA(H1N1)pdm09が持続的に流行し、2010～2011年からの季節性インフルエンザワクチンにはA/California/7/2009 (H1N1)-likeウイルスが含まれていることから、ACIPは2015年7月1日以前に3価または47価のインフルエンザワクチンを2回以上接種した生後6カ月～8歳児では、2015-2016年に必要な接種回数は1回を推奨している。小児および成人において、LAIVとIIVの両方が有効であることが証明されている。一方、LAIVは<2歳および>49歳の集団、添付文書に記載されている禁忌がある集団(アスピリンまたはアスピリンを含む製品を服用している2～17歳の小児、ワクチンへの重症のアレルギー反応を経験した人)、妊婦、免疫抑制状態の人、卵アレルギーのある人、喘息または過去12カ月以内に喘鳴エピソードがあった2～4歳の小児、5歳以上の喘息患者には接種すべきではない。ACIPは、卵アレルギーの既往のある人へのワクチン接種において、軽く調理した卵を摂取可能な人には通常のプロトコールにより摂取し、卵または卵を含有した食品を摂取後に蕁麻疹のみを経験した人には、18歳以上であれば製造過程に孵化卵を使用しないリコンビナントインフルエンザワクチン(RIV3)を接種またはIIVを接種して30分以上経過観察する。卵に対して重症の症状(心血管症状、呼吸窮迫、消化器症状、エピネフリンが必要な反応、救急受診が必要な反応)の経験のある人に対しては、18歳以上であればRIV3を接種し、RIV3が利用できない、または18歳未満では、重症アレルギーの治療経験のある医師によりIIVを接種し、30分間以上経過観察を行う。免疫抑制状態にある人へのワクチン選択および接種時期の詳細のガイダンスは、Infectious Diseases Society of Americaより発行されている。
