Use of Aspirin for Prevention of Recurrent Atherosclerotic Cardiovascular Disease Among Adults — 20 States and the District of Columbia, 2013
成人におけるアテローム動脈硬化性心血管疾患の再発予防のためのアスピリンの使用 ― 20州およびコロンビア特別区、2013年
アテローム動脈硬化性心血管疾患(ASCVD)の既往のある患者では、心筋梗塞、虚血性発作および致死性冠動脈疾患のリスクを抑制するためアスピリンが投与される。今回、2013年のBehavioral Risk Factor Surveillance Systemデータより、アスピリンによる二次性ASCVD予防について分析した。対象は国内20州およびコロンビア特別区(ワシントンDC)の18歳以上の成人175,523名であり、うち21,682名(12.5%)に冠動脈疾患、脳卒中またはその両方に既往があり、社会人口統計学的データおよびASCVDリスク因子に関するデータのない3,698名(17.1%)を除外した17,984名(ワシントンDC:378名~フロリダ州:4,227名)のデータを分析した。アスピリンは70.8%が定期的に使用しており、その目的は心臓発作の予防:93.6%、脳卒中の予防:79.6%、その両方:76.2%であった。また、14.9%が疼痛緩和のため利用していると報告し、4.2%は疼痛緩和のみを目的に利用していた。アスピリンの利用率は男性:76.2%、女性:64.4%、年齢別では65歳以上:75.0%、18~64歳:65.9%、人種別では白人:73.6%、黒人:63.0%、ヒスパニック系:55.6%、その他:64.7%であり、ASCVDリスク因子が少なくとも2つ以上ある場合、利用率が高かった(リスク因子0:54.7%、1:63.4%、2:74.5%、3:76.1%、4:72.7%)。州別のアスピリン利用率は44.3%(ミズーリ州)~71.7%(ミシシッピ州)であった。
