不活化ポリオワクチンの導入と3価経口ポリオウイルスワクチンから2価への切り替え ― 全世界、2013～2016年

1988年、World Health Assemblyはポリオ根絶を宣言し、野生型ポリオウイルス(WPV)の伝播はアフガニスタン、ナイジェリア、パキスタンの3カ国を除くすべての国において中断し、WPV2およびWPV3によるポリオ症例はそれぞれ1999年、2012年11月以降認められていない。これらは経口ポリオウイルスワクチン(OPV)の普及によるものであり、一般的に3価OPV(tOPV)が使用されるが、ワクチン由来のポリオウイルス(VDPV)の出現により、現在、3価の不活化ポリオウイルスワクチン(IPV)の導入およびtOPVから2価OPV(bOPV)への変更が検討されている。IPVは2015年6月24日の時点でWHOに加盟する90カ国にて使用されており、さらに2015年第2四半期、第3四半期、第4四半期、2016年第1四半期、第3四半期にそれぞれ102、32、41、22、1カ国がIPV導入を予定している。現在、OPVは全世界的にtOPVが使用されているが、循環VDPVによるポリオ症例は97％以上が2型VDPV(cVDPV2)によるものであり、また、2014年、cVDPV2によるアウトブレイクがナイジェリアおよびパキスタンにて発生し、2015年3月4日にナイジェリア、3月28日にパキスタンで採取された環境検体でも検出されていることから、WHOのStrategic Advisory Group of Experts (SAGE)は2016年4月を目標に、2型のポリオウイルス株を含まないbOPVへ変更するよう推奨している。bOPVは多くの国々ですでに認可されているが、まだ承認されていない国もあり、世界で同時にtOPVからbOPVへ変更するためにはWHO事前審査に基づきbOPVの使用を許可する必要がある。
