Use of 9-Valent Human Papillomavirus (HPV) Vaccine: Updated HPV Vaccination Recommendations of the Advisory Committee on Immunization Practices

Advisory Committee on Immunization Practices (ACIP)は2015年2月の会議で9価ヒトパピローマウイルスワクチン(9vHPV、Gardasil 9、HPV6、11、16、18、31、33、45、52、58ウイルス様粒子を含む)を定期接種に使用するよう勧告を出した。HPVワクチンは11歳または12歳での定期接種が推奨されており、ACIPは接種歴のない13~26歳の女性および13~21歳の男性に対しても接種を推奨している。HPVワクチンは2価(2vHPV)、4価(4vHPV)があり、9vHPVは2014年12月10日、9~26歳の女性および9~15歳の男性への接種がFDAに承認されている。今回、CDCは2013年10月~2015年2月のACIP HPV Vaccine Work Groupのデータから、9vHPVの有効性、免疫原性、安全性および費用対効果の分析を行った。アメリカではHPV関連腫瘍の約64%はHPV16または18(女性65%、男性63%、年間約21,300例)、10%はHPV31、33、45、52、58(女性14%、男性4%、年間約3,400例)によるものであり、4vHPVと9vHPVの有効性を比較した第III相試験(対象:16~26歳の女性、14,000名)では、9vHPVによるCIN2以上の子宮頸癌、grade2/3の膣および外陰部上皮内腫瘍に対するワクチン有効性は96.7%と報告されている。また、9vHPVに含まれる9種のHPVワクチン型に対する抗体陽転は男女ともに99%以上に認められた。FDAに提出された申請データのうち、6臨床試験、約13,000例を対象とした安全性評価では、安全性プロフィールは4vHPVワクチンと同様であり、発現率は4vHPVに比べ女性(9~26歳)では投与部位反応(腫脹:40.3%、紅斑:34.0%)が高く(4vHPVはそれぞれ29.1%、25.8%)、男性では低かった。9vHPVは4vHPVに比べ1回あたりの接種費が13ドル高いが、費用対効果モデルでは費用削減が可能であり、質調整生存年(QALY)あたりの治療費は25,000ドルを超えなかった。現在、ACIPは11および12歳でのHPVワクチンの接種を推奨しているが、9歳から接種可能とし、22~26歳の男性への接種も推奨する。2vHPV、4vHPV、9vHPVいずれも3回接種とし、2回目は1回目から少なくとも1~2カ月後、3回目は1回目から少なくとも6カ月後に接種する。ワクチン成分に対し過敏性を示す症例には禁忌であり、妊婦への接種は推奨されない。また、接種後、21歳以降65歳までは子宮頸癌のスクリーニング検査を受けることが推奨される。
