Update on Progress in Selected Public Health Programs After the 2010 Earthquake and Cholera Epidemic — Haiti, 2014

2010年の地震およびコレラ流行後の公衆衛生プログラムの進捗に関する最新情報 ― ハイチ、2014年

2010年1月12日に発生した地震でハイチのインフラは壊滅し、推定23万人が死亡、150万人超が住居を失った。さらに震災の10カ月後に発生したコレラのアウトブレイクは、過去に一国内で発生したものの中で最大の規模となった。震災およびコレラのアウトブレイク後には、通常の公衆衛生対策の強化に加え、国内避難民キャンプにおける水や衛生設備の補給、特定の疾病のサーベイランスが優先事項となった。本報告は、2014年末におけるハイチの公衆衛生プログラム、サービスおよびシステムに関する最新情報の一部である。ワクチン冷却設備は、最も被害の大きかった中心都市圏において地震直後の17,000Lから32,500Lへ増加し、2018年までのワクチン備蓄量を確保している。備蓄能力は周辺地域においても2010年に比べ212％増加し、小児の麻疹風疹ワクチン接種率の向上や、5種(ジフテリア、破傷風、百日咳、B型肝炎、Hib)ワクチンおよびロタウイルスワクチンの定期接種への導入につながった。リンパ系フィラリア症に関して、地震後の集団投薬(MDA)キャンペーンにより抗フィラリア薬の服用率は国民の93％に達し、首都ポルトープランスの230万人を含む約800万人が治療を受けた。自治体の58％が5回以上のMDAを完了し、ポルトープランスにおいても2015年と2016年にそれぞれ4回目と5回目の実施が予定されている。コレラの症例数は、2011年の352,033例から2014年10月の15,063例への大幅に減少し、死亡率も2011年からWHOの基準である1％未満を維持している。アウトブレイクの原因となった極度に脆弱な水道衛生インフラに関しては、250名以上の水道衛生技術者を養成し、対応にあたっている。全国調査により、500以上の地域水道システムのうち107以上に塩素消毒が導入された。HIV陽性妊婦の抗レトロウイルス薬服用率は、2008年10月～2009年9月の44％から、地震の年である翌年には32％近くまで低下した。2013年10月～2014年9月の服用率は87％と回復し、同様に抗HIV治療やHIV感染者の結核検査等、他のHIV関連サービスの利用率も回復している。その他、マラリア根絶に向けた検査力の増強、狂犬病サーベイランスの実施、結核診断設備の増強、届出伝染病の定点サーベイランスの拡大等、震災後5年間で一定の改善が見られている。ハイチの例は、資源の限られた国における災害からの復旧の好例となるであろう。
