Vital Signs: HIV Diagnosis, Care, and Treatment Among Persons Living with HIV — United States, 2011

バイタルサイン：HIVと共に生きる患者におけるHIV診断、ケアおよび治療 ― アメリカ、2011年

2011年末におけるアメリカ50州およびワシントンDCのHIVと共に生きる患者およびHIVと診断されている患者数がNational HIV Surveillance System(NHSS)に報告された。19地区(18州およびワシントンDC)のNHSSデータから、2011年、HIV感染症患者は120万人と推定され、うち86％がHIVと診断されており、40％がHIVに対する医療ケアを受けていた。抗レトロウイルス療法(ART)は37％の患者に対して行われ、30％はウイルス抑制に達している。ウイルス抑制率は18～24歳(13％)、25～34歳(23％)、35～44歳(27％)にて低く、45～54歳：34％、55～64歳：36％、65歳以上：37％、人種/民族別では白人：32％、黒人：28％、ヒスパニック/ラテン系：31％、その他：34％であった。2011年、この19地区における新規HIV診断例は15,449例であり、うち12,333例(80％)が診断から3カ月以内に受診したが、13～24歳(73％)および黒人(76％)にて受診率が低かった。ウイルス抑制に達していない839,336例(70％)のうち、20％はHIVと診断されておらず、66％は診断はされたが受診せず、4％は受診したがARTの処方は行われておらず、また、10％はARTを受けていたがウイルス抑制には達していなかった。HIV感染の拡大抑制には早期診断および早期治療介入が重要であり、さらなる努力の必要性が示唆される。
