地域的な麻疹排除への進歩 ― 世界、2000～2013年

2012年、世界保健総会は2015年までにWHOの4地域における麻疹の排除を目的とする世界的ワクチン活動計画を是認、WHOの6地域全てがこの目標を採択した。総会は2015年までに3つのマイルストーンとして、1) 1歳児の麻疹含有ワクチン(MCV)の初回接種(MCV1)率：各国90％以上、各地域80％以上、2)年間発症率：100万人あたり5例未満、3)麻疹による死亡率：2000年から95％以上低下、をたてており、ここではそれぞれの目標達成の状況について報告する。MCV1接種率は2000年：73％、2013年：84％、90％以上に達している加盟国は2000年：84カ国(43％)、2013年：129カ国(66％)であり、2013年における未接種乳児は約2,150万人(インド：640万人、ナイジェリア：270万人、パキスタン：170万人、エチオピア：110万人、インドネシア：70万人、コンゴ民主共和国：70万人)と推定される。MCV の2回目接種率は2000年：15％、2013年：53％であった。症例数は2000年：853,479例、100万人あたり146例から2013年：279,776例(2000年より67％減少)、100万人あたり40例(2000年より72％減少)であり、100万人あたり5例未満の目標に達している加盟国の割合は2000年：38％から2013年：66％(116/176カ国)に増加した。麻疹による死亡例は2000年：544,200例、2013年：145,700例(2000年より75％減少)であった。2015年の目標達成に向けてアフリカ、東地中海および東南アジア(とくにインド)地域における予防接種の強化に必要性が示唆される。
