Interim CDC Guidance for Polio Vaccination for Travel to and from Countries

野生型ポリオウイルスが流行する国を出入国する渡航に関するCDCによるポリオワクチン接種の暫定的ガイダンス

2014年5月5日、WHOは国際的に懸念される公衆衛生上の緊急事態としてポリオの流行に関する声明を発表した。依然としてポリオ常在を認める国が3カ国(アフガニスタン、ナイジェリア、パキスタン)、また過去12カ月以内に野生型ポリオウイルス(WPV)のアウトブレイクまたはWPVの循環を示す環境的エビデンスを認める流行国(カメルーン、エチオピア、赤道ギニナ、イラク、イスラエル、ソマリアおよびシリア)がある。ポリオ感染予防のため、これらの国へ渡航、またはこれらの国から出国する際のポリオワクチン接種に関するCDCおよびAdvisory Committee on Immunization Practicesによる暫定的ガイダンスを示す。医療施設、難民キャンプ、他の人道的援助機関での労働を目的とする渡航を含む全ての渡航者は、推奨されている定期ワクチン接種の状況を確認し、必要な追加接種を受けるものとする。乳幼児に関しては、2カ月齢、4カ月齢、6～18カ月齢および4～6歳にて計4回の不活化ポリオワクチン(IPV)を受けなくてはならない。最終接種はそれまでの接種回数を問わず4歳以上で、前回の接種から6カ月以上の間隔を空けて接種する。乳幼児は定期接種スケジュールに従って接種されるが、推奨される間隔での接種が不可能な場合は1)初回接種を生後6週以降に接種する、2)2回目、3回目の接種は前回から4週間以上空けて接種する、3)3回目と4回目の接種の間隔は最短で6カ月とする。成人の場合、未接種、接種回数が少ない、または接種状況が不明の場合は3回接種する(4～8週間空けて2回のIPV、3回目は2回目から6～12カ月空けて接種)。3回の接種が不可能な場合、3回目は4週間以上の間隔で接種してもよい。また、これらの国々に4週間以上滞在する場合、ワクチンの接種は出発の4～52週間前に受けなければならない。さらに、接種証明、帰国時の追加接種に関するガイダンスも定められ、国内へのWPV輸入阻止対策が講じられている。
