Prevalence and Correlates of Cryptococcal Antigen Positivity Among AIDS Patients — United States, 1986–2012

AIDS患者におけるクリプトコッカス抗原陽性率と相関性 ― アメリカ、1986～2012年

クリプトコッカス髄膜炎(CM)はHIV感染に関連する主な日和見感染症の1つであり、2009年のHIV/AIDS感染生存例におけるCM患者は957,900例、年間平均624,700例が死亡していると推定された。今回、アメリカのHIV感染者におけるクリプトコッカス感染の未診断率を数値化するため、1986～2012年にMulticenter AIDS Cohort StudyおよびWomen’s Interagency HIV Studyにて採取された保存血清検体の分析を行った。CD4 T細胞数100cells/μL未満の1,872検体が分析され、うち55検体(2.9％)がクリプトコッカス抗原(CrAg)陽性を示した。陽性率において、検体採取年、試験の場所、患者の年齢、性別、学歴、人種/民族の違いによる有意な相違は認められなかったが、その他の民族(白人、黒人およびヒスパニック系を除く)にて6.4％と高率であった。また、CD4 T細胞数が50cells/μL超では、CD4 T細胞数50cells/μL以下に比べてCrAg陽性の割合が少なかった(1.7％ vs 4.3％)。
