Two-Dose Varicella Vaccination Coverage Among Children Aged 7 years — Six Sentinel Sites, United States, 2006–2012

7歳児における2回接種水痘ワクチンの接種率 ― 6定点、アメリカ、2006～2012年

2007年、Advisory Committee for Immunization Practices (ACIP)は水痘ワクチンの定期接種を12～15カ月の1回に加え、4～6歳に2回目の2回接種とする勧告を行った。ワクチン接種率を向上させる方法として、接種を入学要件とすることが有効である。2012年の定期接種実施状況の評価のため、水痘ワクチンの2回接種を小学校入学要件とする州の数を2007年と比較した。また、7歳児における2回接種率について予防接種情報システム(IIS)の6定点(ミシガン、ミネソタ、ノースダコタ、ウィスコンシン、オレゴンの5州およびニューヨーク市)のデータを解析した。2回接種を小学校入学要件とする4定点(ミシガン、ミネソタ、ノースダコタおよびウィスコンシン州)では、IISにおける6歳児の2回接種率を、連邦の予防接種事業として評価している幼稚園児の2回接種率と比較した。2回接種を小学校入学要件とする州は2007年の4州に対し、2012年には36州に増加した。6定点における7歳児の2回接種率は2006年に3.6％～8.9％であったが、2009年には平均72.4％、2012年には79.9％～92.0％に上昇し、同様に2回接種を定期接種とする麻疹・おたふく風邪・風疹(MMR)ワクチンの接種率81.9％～94.0％に近接する値となった。IISにおける6歳児の2回接種率は幼稚園児の接種率とほぼ同程度であり、入学要件が1回接種であるニューヨーク市に比較し統計的有意差はないものの、若干高い接種率であった。以上の結果は2007年のACIP勧告から6年間の実質的な進歩を表しており、医療関係者の甚大な接種推進努力が伺える。接種を入学要件とすることで、幼稚園児の水痘ワクチン2回接種率を95％とするHealthy People 2020の目標達成により近づくと考えられる。
