HIV Testing and Risk Behaviors Among Gay, Bisexual, and Other Men Who Have Sex with Men — United States

National HIV Surveillance Systemデータよりゲイ、両性愛者および男性間性交渉者(MSM)における地域別HIV診断率、National HIV Behavioral Surveillance Systemデータより2005年、2008年、2011年における過去12ヵ月以内の男性との避妊具なしのアナルセックス、2008、2011年における異性またはHIV感染状態不明のパートナーとの避妊具なしのアナルセックス、HIV検査歴と感染の認識状態について分析した。2011年、サウスダコタおよびペンシルベニア州を除く州にてHIVと診断された症例の半数以上はMSMであり、過去12ヵ月以内に避妊具なしのアナルセックスを行ったMSMは2005年:48％、2008年:54％、2011年:5％と有意に増加していた。HIV感染状態不明のパートナーとの避妊具なしのアナルセックスを行ったMSMは2008年、2011年ともに13％であったが、HIV陽性を認識していないMSMでは33％とHIV陽性を認識しているMSM(13％)、HIV陰性MSM(12％)の2倍以上高率であった(2011年)。HIV陰性または感染状態不明のMSMでは、67％が過去12ヵ月以内にHIV検査を受けたと報告し(3ヵ月以内:31％、4～6ヵ月:17％、7～12ヵ月:19％)、HIV陽性を認識していないMSMでは5％と低かった。2011年、MSMの18％がHIV陽性であり、その認識率は66％と2008年の56％から増加しているが、HIV陽性例の約1/3が陽性であることに気がついておらず、避妊具なしでの性交が高率に行われていることから、MSMにおけるHIV感染リスク行動の抑制および少なくとも年1回のHIV検査の必要性が強く示唆された。
