Estimating Meningitis Hospitalization Rates for Sentinel Hospitals Conducting Invasive Bacterial Vaccine-Preventable Diseases Surveillance

ワクチンで予防可能な侵襲性細菌性疾患の調査を実施した定点病院における髄膜炎による入院率の推計

WHOによるワクチンで予防可能な侵襲性細菌性疾患(IB-VPD) 定点病院サーベイランスネットワークは、肺炎球菌結合型ワクチンおよびヘモフィルスインフルエンザ菌b型(Hib)ワクチン使用決定に係るデータを提供している。疾患監視サーベイランスのデータを用いてワクチン効果を評価するためには、母集団の変化を一貫した手法で推計する必要がある。2012年、WHOは定点病院管轄区域内の髄膜炎の危険暴露母集団の迅速推計法を開発した。本推計法は国勢調査や住民票などの管理データによる5歳未満児の人口推計および患者の住所情報を含む入院時記録がある場合に適用可能である。入院時診断記録や腰椎穿刺記録を症例サーベイランス結果と照合し、最近2～3年に髄膜炎疑いで入院した5歳未満児を同定する。症例の住所から地理的分布を把握、5歳未満児の入院率を区域の人口数から推計する。髄膜炎疑い例の80％が居住する区域をその定点病院の管轄区域とする。2012年、ガンビアおよびセネガル(両国とも前年に小児定期予防接種プログラムにHibワクチンを導入)にて本推計法を試行した。各首都にある1病院をIB-VPD定点病院とし、5歳未満児の髄膜炎サーベイランスを行った。各国における定点病院の管轄区域は5地区から成り、5歳未満児人口はガンビア130,794人およびセネガル300,842人であった。髄膜炎疑い患者のそれぞれ87％および79％が定点病院に入院し、10万人あたり髄膜炎疑い入院率はそれぞれ42.8および119.8となった。WHOは各国に脳脊髄液の細菌検査に基づく症例ベースの髄膜炎サーベイランスを実施するよう勧告している。2012年には57ヶ国が髄膜炎サーベイランス結果をIB-PVDサーベイランスネットワークに報告した。これらの報告をもとに各地域の比較やワクチン効果の評価が可能となることが期待される。
