Measles — United States, January 1–August 24, 2013

アメリカでは2000年に麻疹の根絶が宣言されたが、その後も国外からの輸入による発症は継続して認められ、2001～2012年の症例数は中央値60(37～220)例/年、アウトブレイクは中央値4(2～16)件/年報告されている。根絶宣言以降、症例数が多かった年は2008年:140例、2011年:220例であるが、2013年は1月1日～8月24日にて16州およびニューヨーク市(NYC)から159例での発症が報告されている。症例は0～61歳[12ヵ月齢未満:18例(11％)、1～4歳:40例(25％)、5～19歳:58例(36％)、20歳以上:43例(27％)]であり、うち17例(11％)が入院治療を要し、4例は肺炎と診断された。死亡例は認めていない。157例(99％)が輸入例(2例は感染源不明)であり、遺伝子型はD8(47例)、B3(6例)、H1(4例)、D9(3例)、D4(2例)であった。多くの症例がワクチン未接種例(131例、82％)または接種状態不明(15例、9％)であり、13例が接種を受けていたが、麻疹・おたふく風邪・風疹(MMR)ワクチンの2回接種例は3例であった。アウトブレイクは現時点で8回報告され、症例数は3～58例、最も大規模のアウトブレイクはNYC、次いでノースカロライナ州(23例)にて発生し、現在、テキサス州にて進行しているアウトブレイクでは8月24日時点で20例が確診されている。これらのアウトブレイクはワクチン未接種例にて拡大しており、ワクチンの接種が感染の拡大には重要であることが示唆される。
