Building Laboratory Capacity to Support the Global Rotavirus Surveillance Network

世界的ロタウイルスサーベイランスネットワークを支援する研究室の受け入れ能力の整備

2006年、WHOは2種類のロタウイルスワクチン(ロタリックス:1価ワクチン、2回接種、ロタテック:多価ワクチン、3回接種)を認可し、全ての国に対し予防接種プログラムへの導入を推奨している。2008年、Global Rotavirus Surveillance Network (GRSN)が設立され、その基礎となるGlobal Rotavirus Laboratory Network (GRLN)がロタウイルスの診断的検査および異なる国や地域で分離されたウイルスの遺伝子型の分析を行っている。2013年4月の時点でこのネットワークには107ヵ所の拠点病院検査室および36ヵ所の国立研究所、9ヵ所の地域の標準検査室(RRL)および1ヵ所の世界的標準検査室(GRL)が属しており、拠点病院が診断基準を満たす急性胃腸炎により入院した5歳未満の小児例を登録し、国立研究所は登録された小児例の便検体の検査、保存および遺伝子型の分析をサポートしている。RRLは国立研究所に対し検査法の選択および遺伝子型の分析方法など技術的なサポートを行い、さらにGRLは遺伝子型分析の指導や検査結果の質の保証などRRLが必要とし、要求する検査に関する活動をサポートしている。GRSNを通じてデータを報告した国は2008年:44ヵ国から2011年:64ヵ国に増加し、この期間に拠点病院は132から185ヵ所、年間の登録小児数は41,414例から48,947例に増加している。便検体からのロタウイルス検出率は36～41%であり、2009～2011年のロタウイルス陽性検体は14,902検体であり、検出された遺伝子型は5種類([G1P(8)、G2P(4)、G3P(8)、G4P(8)、G9P(8)]であった。ネットワークから提供される情報はロタウイルスワクチンの導入を検討する上で重要であり、ワクチンの導入による影響のモニタリングにも有用であると思われる。
